5. Gender (male or female)
6. Date of birth (YYYY/MM/DD such as 1970/04/02 for April 2, 1970) and age on June 1, 2019.
7. If there are companion(s), all the No.2-6 data for each.
8. The address where we should send the documents by EMS (including phone number)
9. Your itinerary in the following format
   (For EACH flight please write date of flight, departing airport, flight number, arriving airport.)
   (To have a right information please purchase the flight tickets before filling this part.)
